Launched in 2018 to provide access to resources needed to promote wellness, Baal Perazim Wellness & Health Services has six different locations in the Chicagoland area, as well as several mobile programs. Our guiding principal is to provide care that impacts the Mind, Body, and Soul.
Baal Perazim focuses upon Health Equity strategies that promote wellness and health for all, regardless of race, age, gender or other social determinants.
We believe that the assets needed to promote wellness should be distributed equally to all Chicagoans. We believe that the lack of appropriate resources facing many Chicagoans has led to profound and unacceptable health disparities.

Baal Perazim (ba-al peh-raw-tseem) means “breakthrough”, and is the site of an Old Testament victory gained by David over the Philistines (2 Samuel 5:20).
