Direct Payments

Baal Perazim Wellness & Health Services Inc. accepts most major credit cards and is happy to provide no contact payment options by telephone. For questions about your balance or to make a payment, please call our Customer Service Specialist at 773-296-2400. Payment is usually due the date of service, unless other arrangements have been made in advance. Self-pay patients are responsible for the entire amount of the bill. Patients participating in health plans are responsible for any amounts for any amounts not covered by their insurance. Co-pays are collected at each visit.
