At The Wellness Homes of Chicago, we believe that everyone deserves access to quality primary and behavioral health care. We understand that the cost of care can be a barrier, especially for those who are underinsured or uninsured. That's why we offer a sliding fee discount program based only on family size and annual income to make certain core services more affordable.
How Does It Work?
Eligibility is determined solely by family size and total annual income, enabling us to offer sliding-scale fees that reflect the needs of our community. Our program is designed to ensure you can access the care you need without undue financial stress. We're committed to working with you to create a payment plan that aligns with your budget.
Family size is defined as the number of individuals living together in the same household who are financially dependent on the same income.
Proof of income is required. For every individual in your household who receives income, you will be required to provide the two (2) most recent pay stubs or equivalent proof of income.
How to Apply
To find out if you qualify for our sliding fee discount program, simply complete a brief application by clicking the link to the electronic form below. A completed application allows us to assess your eligibility and determine the payment level that best fits your financial situation.
Once you've completed the form, it will be sent back to our office. Our team will review the application to determine your sliding scale eligibility and get you scheduled for care.
